Permohonan Sertifikat Online FIK UI

FoN UI Online Certificate Application
Step 1

Pilih Template

Pilih template sertifikat yang disukai lalu unduh dan sunting template tersebut.

Step 2

Unduh Db dan Draft

Unduh template Database Penerima Sertifikat dan Draft Surat Permohonan.

Step 3

Persiapkan Berkas

Sunting berkas-berkas yang sudah diunduh pada Step1 dan Step 2.

Step 4

Isi Form Pengajuan

Lakukan pengisian Form Pengajuan Sertifikat dengan mengikuti instruksi yang ada pada form tersebut.

Unduh berkas ini, Isikan data peserta pada kolom Nama Peserta, Institusi Peserta, Poin SKP, dan Kepesertaan,.
Jangan menghapus/menambahkan kolom dari template yang ada.

Draft Application Letter

Unduh berkas ini, jika jenis surat Anda untuk unit kerja internal FIK UI.
contoh: Divisi, Departemen, dll yang memiliki kode penomoran untuk nota dinas.

Unduh berkas ini, jika jenis surat Anda untuk selain unit kerja internal FIK UI.
misalnya: Organisasi Mahasiswa, Institusi Lain yang bekerja sama dengan FIK UI, dll yang tidak memiliki kode penomoran nota dinas karena biasanya mereka memiliki kode nomor dan/atau kop surat sendiri.

Eselonisasi Jabatan

Berkas ini merupakan informasi tambahan.
Dalam berkas ini berisi informasi Eselon dan Pejabat apa saja yang ada pada eselon-eselon tersebut.

3. Persiapkan Berkas untuk Permohonan Penerbitan Sertifikat

      Prepare Files for Certificate Issuance Application

  1. Template Desain yang sudah diunduh lalu disunting dan simpan sebagai file PNG,
  2. Data Penerima Sertifikat diinputkan ke Template Database Penerima Sertifikat,
  3. Surat Permohonan dibuat sesuai dengan draft Surat Permohonan dan sudah ditandatangani oleh pejabat terkait,
  4. Surat Informasi Kegiatan (Surat Undangan Kegiatan atau Surat Tugas Panitia) sudah dibuat dan ditandatangani oleh pejabat terkait.
  1. Downloaded Design Template then edited and saved as PNG files
  2. Certificate Recipient Data is inputted into Certificate Recipient Database Template,
  3. The Application Letter is made in accordance with the draft Application Letter and has been signed by the relevant official,
  4. Activity Information Letter (Activity Invitation Letter or Committee Assignment Letter) has been made and signed by the relevant official.

4. Mengisi Form Pengajuan Sertifikat

      Fill in the Certificate Application Form

Klik tombol berikut, lalu ikuti panduan pada Form Pengajuan Sertifikat dan unggah file sesuai instruksi pada form.
